Ballpark Fee Schedules For Fertility Treatment Cycles


Cost of Ovulatory Monitoring Without Meds
Cash patient will pay $615-$800


Cost of Ovulatory Monitoring With Clomid
Cash patients will pay $775 - $960


Cost of FSH Intercourse
Cash patients pay as they go for each visit will pay $1,580 -$2,100.
A global prepayment is available for cash patients. The payment is due on patients baseline sonogram in the amount of $1,495. Patient has the option to pay as they go or pre-pay globally.


Cost of Timed Intrauterine Inseminations
Cash patient will pay $750 - $1,400 depending on whether patient has on or two inseminations.


Cost of Intrauterine Insemination With Clomid

Cash patients will pay $910 - $1,500 depending on whether patient is inseminated once or twice.


Cost of FSH Intrauterine Insemination
Cash patients who pay as they go will pay $1,790 - $ 2,655 depending on number of sono/e2s and if they are inseminated once or twice.
Global pre-payment available for cash patients. Payment is due on baseline sonogram in the amount of $1,580. Pre-payment only covers one IUI, if patient needs a second insemination there would an additional charge of $185. The charge covers the SA, sperm prep, and insemination charge.
Budget $2,000 -$3,000 for medications which patient will fill at pharmacy.


Cost of In Vitro Fertilization Out of Town Patients or Pre-payment / Time of Service
Due prior to receiving treatment calendar.
Patient will pay one of the following fees:

  1. For procedure only w/o ICSI or freeze $4,925.
2. For procedure & freeze $5,675.
3. For procedure & ICSI, $6,425
4. For procedure, ICSI, & freeze $7,175


Patient must pay for any sonograms or blood work done at the time of service.
Medications will purchase through pharmacy.
Patient should budget at least $3,000 to 4,000 for meds.
Patient will need (2) two week lupron kit at $350 apiece, and between 30-40 FSH ampules which run $50 - $65 per ampule.


Cost of In Vitro Fertilization Global

Patient will pay on the following fees:

  1. For entire cycle w/o ICSI or freeze $6,190
2. For entire cycle with freeze $6,940
3. For entire with ICSI $7,690
4. For entire cycle with ICSI and freeze $8,440

Injection teaching class is mandatory prior to starting injections.
Group injection class is held every Thursday evening at 6:00 p.m.
The cost of the class is $25.
Patients who need a private injection teaching class will pay $50.
Patient should budget at least $3,000 to $4,000 for meds.


Cost of Frozen Embryo Transfer (CASH PATIENTS)
Patient is required to pre-pay for procedure the day patient has sonogram to start lupron. The fee is $2,545 and pays for cycle management fee, embryo thawing, transfer, and facility fee.
Sonograms and E2s are included in payment.


Cost of Donor Embryo
Patient pays $1,000 for donor embryo screening.
Patient pays $2,545 for frozen embryo transfer and sonograms.
Payment due for FET and Donor Embryo prior to receiving treatment calendar.


Cost of Egg Donation

Patients first step is to pay the administrative fee of $1,500 and recruitment fee of $1,000. This fee is non-refundable. Payment is due prior to entering the egg donation program. Before accepting payment for this, we must get the patient to sign and notarize the Egg Donation Administrative Fee Policy form and the patient must also sign the Estimated Fee Schedule for this cycle.
Payment for donor cycle is due prior to receiving treatment calendar. Patient will pay the following fees:

  1. Donor's medications $3,300
2. Donor's cycle fee (e2, sono's, ov, injections, post op, pain meds, &     antibiotics) $2,600
3. Donor retrieval, lab, and transfer $2,475
4. Facility fee $1,700
5. Embryo freezing $750
6. Total due for cycle including recipient sonos, E2, and cycle     management $12,220
7. If a candidate for ICSI $1,500
8. Total due for cycle with ICSI including recipient sonos, E2, and cycle     management $13,720.
9. Donor compensation must be paid in cash in the amount of $2,500.


Refer to Egg donor fee schedule for pre-cycle requirements for recipient.


Cost of Testicular Biopsy / Sperm Aspiration

Fee due to the Center for Assisted Reproduction must be paid prior to receiving a treatment calendar.
The following fees will be due to CAR:

  1. Lab fee $800
2. Operating room fee $1,200
3. $600 for anesthesia.


The urologist will bill surgeon's charge.


Cost of Tubal Reversal
Payment for tubal reversal surgery is due 14 days prior to procedure.
The following fees will be due:

  1. Facilities charge for minilaparotomy includes laparoscopy if needed     $2,400.
2. Dr. Doody's surgeon fee $3,000
3. Total due to CAR $5,400
4. $800.00 for anesthesia
